Insurance & Billing
This office currently accepts MasterCard, Visa, personal checks, and cash for co-payments. All co-payments are due at the appointment time. We reserve the right to refuse service when patients do not make their co-payment.
Insurance
We are currently accepting new patients who subscribe to select PPO and Physicians Associates (PA), affiliated PA/HMO insurances, Healthcare Partners, and Medicare with a PPO supplement. Please call our office to see if we accept your insurance. If your insurance covers your treatment, you will receive the benefit of reduced personal costs. Insurance policies vary, and we will review your insurance to determine the appropriate course of action.
We also accept self-pay patients. Payment for self-pay patients is due at the time of service. All insurance co-pay fees are due at the time of service. As a service to our patients, we will bill insurance companies for service. Deductible and unpaid portions of the bill remaining after insurance adjustments will be the patient’s responsibility.
Please remember that insurance is a benefit, and if your benefits change during the course of treatment, you are accountable for all charges.
Billing
All balances on your account, after insurance payments and adjustments, are payable within 30 days of receipt of your statement. All accounts over 120 days past due are subject to referral to our collections agency and reported to all three credit bureaus. Please make every effort to pay your bills on time, so that we can continue to provide the best service possible.
