Our Mission
Huntington Family Medicine’s mission is to provide personalized, effective, prompt and caring medical service to our patients. Our doctors and staff are fully trained and educated to assist you with your health concerns. We constantly keep abreast of new medical issues that affect you and your family. We have created an environment of compassion and professionalism to make your visit as comfortable and pleasant as possible. We look forward to your good health and a long and healthy relationship with you.
Appointments
Your scheduled appointment time is reserved specifically for you. We request 48-hours notice if you need to cancel your appointment. We are aware that unforeseen events sometimes require missing an appointment, but with your help and thoughtful consideration for informing us of cancellation we will be able to allocate the time to other needy patients.
Insurance
Currently we accept most insurance policies. Please verify with our staff at the time of appointment. We also accept self-pay patients. Payment for self pay patients is due at the time of service. Your co-pay portion according to your insurance policy must be paid at the time of service. As a service to our patients, we will bill insurance companies for service. Deductible and unpaid portion of the bill after insurance adjustments will be the patient’s responsibility.
New Patients
We are currently accepting new patients that are subscribers to select PPO and Physicians Associates (PA), affiliated PA/HMO insurances, Healthcare Partners and Medicare with a PPO supplement. Please call our office to see if your insurance is accepted by us.
Other Policies
- Patients will be charged a $25 fee for copies of medical records.
- Please allow 48 hours for pharmacy refills.
- Please turn off your cell phone during examinations.
